Keith L. Evans, 79, of Warsaw, Ind., died at 11:30 a.m. Thursday, Nov. 1, 2012, in Mason Health and Rehabilitation Center, Warsaw. He was born April 24, 1933, in Kosciusko County to Lawrence and Ellen (Bartholomew) Evans.
