Thursday, Jan. 31, 1:09 p.m. US 30. Driver: Reginald E. Banks, 40, West 87th Street, Chicago, Ill. Banks, a semi driver, lost control due to driving too fast for weather conditions. Banks went into the median causing undercarriage damage to the left fuel tank of his truck. Diesel fuel spill caused a hazmat clean up of the Warsaw Fire Department. Damages: up to $2,500
