WARSAW — At approximately 3:32 p.m. Monday, Oct. 24, the Kosciusko County Sheriff’s Department, Mentone EMS and Atwood Fire responded to a single-vehicle accident at 5920 Crystal Lake Road, Warsaw.

The vehicle exited the roadway and hit a tree.

The driver of the vehicle had to be extracted from the vehicle and was transported.
