LEESBURG — Shortly after 5 p.m., today, Feb. 23, the Kosciusko County 911 Communications Center received multiple reports of a head-on collision, in the 8400 block of North SR 15. The Kosciusko County Sheriff’s Office, Milford Police and Fire Department and Lutheran EMS were dispatched.

Initial arriving units determined that two vehicles, both of which sustained heavy front end damage, were involved in the collision. The Kosciusko County Fatal Accident Crash Team was requested at the scene.

The preliminary results of the investigation indicate that a 2002 Honda Civic, driven by Keshawn Smith, 18, Goshen, was traveling southbound on SR 15. The Civic appears to have crossed the center line, into the path of travel of a northbound 2013 Ford Expedition, driven by Ramiro Mendoza Fajardo, 30, Goshen. The Expedition was towing a 2014 Jeep Commander, on a car dolly.

The Kosciusko County Coroner’s Office pronounced Smith deceased at the scene. Ramiro and his passenger refused medical treatment.

This crash remains under investigation with the Kosciusko County Sheriff’s and Coroner’s Offices.
