David Van Dyke, 69, Warsaw, passed away at 1:35 p.m. in his residence Thursday, Feb. 7, 2019. 

He was born on Oct. 11, 1949, in Kosciusko County, Indiana, to Harold Eugene and Olive Mae (Slife) Van Dyke. 

He is survived by a fiancé; Denise Wawrzyniak.
