Charles D. “Dwayne” Jarvis, 53, Warsaw passed away at 5:02 p.m., Monday, April 16, 2018, in Lutheran Hospital, Fort Wayne.

A graveside service will be held at 11 a.m., Saturday, Aug. 11, 2018, at Lake View Cemetery, Silver Lake. Reverend Glen Hughes will officiate the service.

Interment will be at Lake View Cemetery, Silver Lake.
