MIDDLEBURY — The Indiana Occupational Safety and health Administration has fined the Winnebago plant in Middlebury $10,000 for workplace violations after an employee was run over and killed by a forklift in April.

The coroner said 54-year-old Rick Schlabach died of blunt force injuries in the accident at Winnebago Industries on 14th Street.
