Phyllis M. Jernstrom, 86, Plymouth, passed away on June 6, 2016.

She was born on Feb. 27, 1930.

Visitation will be from 2-5 p.m. Sunday, June 12, at Van Gilder Funeral Home, Plymouth. Services will follow at 5 p.m. Sunday, June 12. A burial ceremony will be held at 10:30 a.m. Monday, June 13 at New Oak Hill Cemetery, Plymouth.
