Sally A. Phillips, 70, Leesburg, passed away at 11:50 a.m. on Monday, May 30, 2016, at Parkview Regional Hospital in Fort Wayne. She was born on Jan. 10, 1946 in Warsaw, to Eldon and Louise (Watkins) Clayton.

She grew up in North Webster, graduated in 1964 from North Webster High School, and attended Manchester College in North Manchester. She was married on April 10, 1982 in North Webster to Jerry J. Phillips who survives and lived most of her adult life in the Fort Wayne, Huntington, and Leesburg areas. She was formerly employed at St. Joseph Hospital, Park Center, Lopshire Flowers and Armstrong Flowers all in Fort Wayne.

She formerly attended the Church of God in North Webster.

She is survived by her husband: Jerry Phillips, Leesburg; two step-sons: Jeffrey (Cindy) Phillips; and Jason Phillips, both of Huntington; four step grandchildren; three sisters: Betty Clayton; Dee Stetzel, both of Albion; and Eldonna (Randy) Warren, North Webster; a brother: David (Judy) Clayton, Kimmell.

She was preceded in death by her parents and sister: Rosalind Clayton.

Cremation will take place at Cremation Services of Kosciusko County, 4356 N. St. Rd 15, Warsaw. Visitation will be from 6 p.m.–7 p.m. on Thursday, June 16 at Owen Family Funeral Home, St. Rd. 13 & CR 500N, North Webster.  Memorial services will follow at 7 p.m. on Thursday in the funeral home.
