KOSCIUSKO — Only five Indiana agencies were selected by Deyta Analytics to receive the national honor of being named as a 2015 HHCAHPS Honors Award Recipient.

HHCAHPS Honors is a prestigious award recognizing home health care agencies that continuously provide quality care as measured from the patient’s point of view. Award winners were judged on patient surveys.ȊWe are so honored to be recognized for our dedication to quality patient care.

We are the only agency in northern Indiana receiving this prestigious awardȋ, said Rick Paczkowski, Kosciusko Home Care & Hospice Administrator. Kosciusko Home Care & Hospice has been serving local residents for almost 40 years. KHCH is the only local non-profit agency providing home health care and hospice in Kosciusko County.
