Patricia J. Johnson, 56, Columbia City, died at 5:20 p.m. Tuesday, Oct. 20, 2015 at the Hospice Home, Fort Wayne.

She was born Aug. 6, 1959, in Elgin, Ill., a daughter of Loren E. and Virginia M. (Merritt) Switzer. Her formative years were spent in Elgin. She graduated from Elgin High School. In 1982 she moved to Griffin, Ga. until 1988 when she moved to the Columbia City and Fort Wayne areas, moving to her present home in 2005.

She worked as a CNA for 30 years, a QMA for seven years, Physical Therapy and Occupational Therapy Care for 10 years and activities assistant for three years.

Surviving relatives include two children: Nicole M. Fay, Elgin, Ill.; and Ryan K. Johnson, Fort Wayne; twin grandchildren: Aden and Colton Johnson; four sisters: Karen Burnett, Elgin, Ill.; Marcella Switzer, Texas; Debbie Ward, Georgia; and Rebecca Switzer, Loves Park, Ill.; and three brothers: Michael Switzer, Rockford, Ill.; Eddie Switzer, Loves Park, Ill.; and Virgil Switzer, South Elgin, Ill. She was preceded in death by her parents and her twin brother: Patrick Switzer.

There will be no visitation or service. Burial will take place at a later date at Lakewood Memorial Park in Elgin, Ill.

Preferred memorials are to Shriners Hospitals for Children.
