All youth in grades K-12 who participate in 4-H are required to complete the Elkhart County 4-H enrollment form annually. Students in grades 3-12 wishing to enroll in Kosciusko County will need to pay a $20 fee for 4-H. For students in kindergarten through grade 2 interested in Mini 4-H, the fee is $5.

The 4-H enrollment form must be completed and submitted along with the enrollment fee to the Kosciusko County Purdue Extension office by Jan. 15, 2016. This is the statewide deadline for 4-H enrollment.
