Jay A. “Bear” Horn, 59, Warsaw, died at 1:07 a.m. Monday, June 15, 2015, at the Warsaw Meadows.

He was born Feb. 19, 1956, in Kosciusko County, to Don and M. Jeanne (Miller) Horn.

He is survived by his son: James Horn, Warsaw; and his sister: Christine Horn, Carmel.
