Jzamya Brooke Carrizales, infant daughter of Melissa Carrizales, passed away on Tuesday, Oct. 14, 2014, in South Bend.

She was born on Oct. 13, 2014.

She is survived by her mother: Melissa Carrizales, Ligonier; her grandparents: Aracelia and Andres Carrizales,Ligonier; and uncles: Andres, Jr. (Berenice) Carrizales and Michael (Aneida) Carrizales.

A cremation committal will take place at Legacy Cremation & Remembrance Center in Ligonier
