The K21 Health Foundation is currently accepting applications for the first round of grants in 2013. Completed applications must be received no later than 4 p.m. Friday, Feb. 1, to be considered during the cycle.

Non-profit organizations with programs dedicated to the health and wellness of Kosciusko County residents are encouraged to apply. All applicants will be required to submit the redesigned grant application which is available on the K21 website.

K21 Health Foundation exists for the benefit of Kosciusko County citizens to ensure health care services are provided, and to advance prevention and health lifestyles.
