The Elkhart County Crash Team is investigating a fatal accident that happened near Milford Saturday night.

According to the Elkhart County Sheriff’s Department, Chandra Fernandez, 33, was eastbound on U.S. 6 at about 6 p.m. when her minivan left the highway and rolled. Fernandez was pinned underneath the vehicle, which came to rest in a cornfield. She was pronounced dead at the scene.

Authorities say Fernandez was wearing a seatbelt.

The accident is still under investigation.
